To make your first visit as smooth and efficient as possible, please download and complete your new patient forms prior to your appointment. Bringing completed forms helps us spend more time focusing on your care. If you have any difficulty accessing the forms, our team is happy to assist you.
Your First Visit
Your initial visit may include a comprehensive oral examination, full mouth digital X-rays, oral cancer screening, oral hygiene guidance, and a professional cleaning when appropriate. You will also have a detailed consultation with one of our doctors to discuss your needs, expectations, and treatment options.
During your visit, we will answer all of your questions and provide a clear understanding of your oral health. A financial consultation will also be included to help you plan your treatment in a way that is both effective and manageable.
Insurance and Billing
We are out-of-network providers, however we will submit claims on your behalf for reimbursement and help you maximize your benefits. We accept many major dental insurance plans, including Delta Dental, Humana Dental, MetLife, Aetna, and United Concordia. Our team will clearly explain your coverage and financial responsibilities so there are no surprises.
